Guidance Committee Form
Guidance Committee Forms are due by the end of a student's first quarter/semester. The student should meet with the Guidance Committee to plan coursework. Courses can be changed, but each committee member will need to intial each change. Please list the term and year for each course taken on the form.

M.S. Advancement to Candidacy
M.S. students are eligible and expected to file for advancement to candidacy no later than their fourth quarter of residence, after completing at least half of their required coursework. M.S. students must pay a $55 fee at the Cashier's Office and submit the appriopriate form (M.S. Plan I Candidacy Form or M.S. Plan II Candidacy Form) to the GGE office.
If you are not able to physically go to the Cashier's Office, checks made payable to UC Regents can be mailed to the GGE Office along with the appriopriate form.
Pre-Qualification Exam Forms
Ph.D. students should request a meeting with their Guidance Committee to complete the Pre-Qualification Exam Form. It is highly recommended to meet at least one quarter/semester prior to the examination date. All coursework must be completed by the date of the exam. Please submit this form to the GGE office for evaluation 60 days prior to your exam. You will also need to submit the Graduate Studies Qualifying Exam Application to the program as well.
Ph.D. Advancement to Candidacy
After passing your qualifying exam, you apply to advance to candidacy by paying a $90 fee at the Cashier's Office and submitting Ph.D. Candidacy Form to the GGE office.

Within 6 months after passing the QE, Ph.D. students must submit to the GGE office a formal dissertation proposal signed as approved by each member of the dissertation committee.
Before filing your dissertation, host your exit seminar. Submit a copy of your flyer advertising the exit seminar to the GGE Office.
